What info might we want to capture?

How can we make it minimalist but informative so that people are glad to do it and not overwhelmed?

I am considering just making it a few questions. Like

  1. Title - make it meaningful and simple
  2. Describe the SCRAP.
  3. Why do you believe it is SCRAP? What did it get in the way of? What problems did it cause? Share any examples
  4. Any other notes or comments?
