The South Park reference in the middle is cool and can stand alone. The design doesn’t match the outer circle though so it kind of looks out of place. I suggest making them 2 separate diagrams.
To bridge the two you could state something like: An unclear Phase 2 (purpose, implementation, etc.) leads to this perpetual/downward cycle.

I’d like to keep 1 diagram because the story is as you say….the ? leads to the cycle, and we see a lot of that right now. if I made it a sort of 2 frame thing where the ? has an arrow entering the cycle….would that be better? Other ideas?
Maybe keep the South Park image in the middle and put a pithy caption under the whole image.
